CHILDREN’S PRIVACY

The Service is general audience and intended for users 18 and older. We do not knowingly collect Personal Information from anyone younger than age 18.

HIPAA; BUSINESS ASSOCIATE AGREEMENT

As we are a technology provider that helps to connect potential patients (“Users”) with qualified doctors (“Healthcare Provider”), we do not have a direct relationship with patients. If we receive any inquiries or requests from Users about their Personal Information, we will direct those inquiries or requests to the Healthcare Provider (defined below).

Because AccidentDoctors911 links our Users to Healthcare Providers by providing appointment requesting services, AccidentDoctors911 is a “business associate” of “covered entities” under § 160.103 the Health Insurance Portability and Accountability Act of 1996 (“HIPAA“).

To the extent that you disclose any “protected health information” (as defined pursuant to the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”)) to us, your disclosure shall be subject to the Business Associate Agreement. We make no representation or warranty that the Business Associate Agreement is necessary for the compliance by you with HIPAA or other applicable law or regulation regarding any such protected health information.  You are fully responsible for your actions with respect to any such protected health information, and agree to indemnify, defend and hold harmless AccidentDoctors911 for and against any violations by you of HIPAA or any other applicable law or regulation regarding any such protected health information. In the event of a conflict between this Policy and the Business Associate Agreement, the Business Associate Agreement shall control. 

INFORMATION WE COLLECT ABOUT YOU AND HOW WE COLLECT IT

Healthcare Providers must first register for an account on the Website. Users are not required to register for an account. In registering for an account, we collect certain information as described below.

Information You Provide to Us or Received by Us on Your Behalf

The categories of information we collect vary depending on whether you are a User or Healthcare Provider.

From our Users, we collect: 

  • Personal Information: In the course of registering on our Website, filling out a contact form, and using our Services, we may require or otherwise collect information that identifies you as a specific individual and can be used to contact or identify you (“Personal Information”). We collect this information on behalf of our Healthcare Providers to facilitate appointments between users and medical professionals. Examples of Personal Information we collect for these purposes include your first and last name, telephone number, email address.
  • Appointment Information: We may collect information related to your health appointment, also on behalf of Healthcare Providers, such as your availability, calendar and meeting information.

From our Healthcare Providers, we collect:

  • Personal Information used to register an account on our Website, which includes your: name, phone number(s), e-mail address, and office address.
  • Professional Information: In the course of using our Services, we may ask for your position and role, area of practice, company/business affiliation and information, biographical information, employment history, licensure information, professional credentials, calendar and scheduling details, and other information related to your practice.
  • Payment Information: We may also collect your payment information, such as credit card, billing address, and other financial information necessary to purchase or otherwise use our Services. By submitting your Payment Information, you consent to us providing your Payment Information to those Authorized Third-Party Service Providers, such as Stripe, as are reasonably necessary to support and process your transactions as well as your credit card issuer and banking institution.

For both Users and Healthcare Providers, we collect:

  • Non-Identifying Information: We also may collect non-identifying or non-personal information, such as zip codes, demographic data, time zone, publicly available data, and general information regarding your use of the Service (“Non-Identifying Information”).

Information Collected Automatically

As you navigate through and interact with our Website, We and our third-party service providers, including analytics and third-party content providers, may automatically collect certain information from you whenever you access or interact with the Service, such as.

  • Usage Information. Details of your visits to our Website, including which links you clicked on, content response times, location data, logs, and other similar communication data and statistics about your interactions.

    • We share the personal information of Users with Healthcare Providers to provide the Services. For example, we share the contact details and appointment information of a User with a Healthcare Provider.
  • With Users
    • We share Personal Information and Professional Information of Healthcare Providers with Users to enable Users to select an appropriate Healthcare Provider.
  • With our service providers and network partners:
    • We employ third party companies and individuals to provide Services on our behalf, to perform Service-related operations (e.g., without limitation, maintenance services, database management, web analytics, payment processing, fraud detection and improvement of AccidentDoctors911’s features) or to assist us in analyzing how our Service is used. These third parties may have access to your Personal Information in order to perform these tasks on our behalf.
  • For corporate transactions
    • Accidentdoctors911.com may share information, including Personal Information, with any current or future subsidiaries or affiliates, primarily for business and operational purposes.
